Phyllis Kaye Cook, 59 of Menifee, California passed away on Sunday, June 28, 2015, at Menifee Valley Medical Center in Menifee, California .

She was born on Sunday, November 13, 1955 in Rockford, Illinois, she was the daughter of the late Virgil Box and the late Reather Greer Box.  She was the wife of Charles Cook.

Surviving are son, Brand Box, daughter, Candace Griffin; 2 grand children.

A celebration of her life will be from 3 until 5 p.m.  Saturday, August 15, 2015, at Bethel Baptist Church, 724 Harrison Ave, Rockford.
